To inform ongoing quality improvement efforts, the Tennessee Department of Education (TDOE) has requested that UT SWORPS conduct a student survey. The survey is being administered with all extended learning programs across the state.
The following consent documents must be completed by the families and returned to the Extended Learning Program before a student can complete the survey. Please do not change the language of the consent form as they have been approved by TDOE:
The online surveys are designed to be administered to students enrolled in 3rd -12th grade:
- Elementary School Survey (3rd-5th grade)
- Middle & High School Student Survey (6th-12th grade)
- Encuesta para estudiantes de escuela primaria – 3-5 Grado
- Encuesta para estudiantes de secundaria – 6-12 Grado
Student responses will be anonymous, and the information will only be available to UT SWORPS evaluators. Each extended learning program will receive a report summarizing their students’ responses, but no individual responses will be reported, and no students will be identified in reporting.
