(3/27/26) To inform ongoing quality improvement efforts, the Tennessee Department of Education (TDOE) has requested that UT SWORPS conduct a student survey to collect their opinions of the program. The survey is being administered with all extended learning programs across the state.
The following consent documents must be completed by the families and returned to the Extended Learning Program before a student can complete the survey. Please do not change the language of the consent form as they have been approved by TDOE:
The online surveys are designed to be administered to students enrolled in 3rd -12th grade. You can view the survey questions using these links:
The survey is anonymous and designed to protect students’ privacy. Students will not be asked to put their names on the survey. They may skip any questions they don’t want to answer or may stop taking the survey at any time. The information gathered from this survey will only be available to UT SWORPS evaluators.
Each extended learning program with 5 or more respondents will receive a report summarizing their students’ responses, but no individual responses will be reported, and no students will be identified in reporting. Please feel free to contact sworpshelpdesk@utk.edu with any questions.
